Transportation Permission Form

If you or your child will be giving a ride to any student who is not a member of the driver’s immediate family, you need to click on the form below.  The second page of the form needs to be completed and returned to the school before a ride can be provided to non-family members.

Please note that your auto insurance policy must provide liability limits of at lease $100,000/$300,000 along with a minimum property damage limit of $50,000.
